National Health Law Program’s (NHeLP) has created the survey (link below) to learn more from families about their experience with the California Children’s Services (CCS) program and the CCS appeal process.
NHeLP is a national non-profit. They are not a government agency and really appreciate you taking the time to answer their survey questions. Any responses to this document will remain anonymous. All questions are optional and you can skip any questions for any reason. If you have more than one child using CCS services, please submit separate survey responses for each child. Please complete the survey by December 31, 2022. If you would like to be eligible for a random drawing of a $25 visa gift card (5 respondees will be randomly chosen), please enter your email address at the end of the survey.
